If you are considering the PDM Network, please click on the "Contact PDM" link, and request a "Credential Application" and "Personal Reference" form. We will send these to you via Email or in the regular mail, if you prefer. Fill out the "Credential Application", and send it back to us. Please give one copy of the "Personal Reference" form to each of the two references you put on your application. They will fill this out and send it back to PDM. We will contact you for an initial interview with our apostolic board, either in person, on Zoom, on Facetime, or using Google Meet, once your paperwork has been received.
